Facility Application Form - Athletics	Use this form to reserve recreation facilities for a multi-week sports season
Waiver Release	Collect a waiver from each adult participant
Waiver Release - Minor	Collect a waiver for each minor participant
Rules and Regulations	Users must read and agree to the town's field/facility and park rules and regulations prior to any application being approved by the Parks and Recreation Commission
